Individual Nutrition Education Sessions
Personalized Nutrition Education. One Person at a Time.
The Individual Nutrition Education Curriculum is designed to provide standardized, evidence-based nutrition education that can be delivered during one-on-one patient encounters. Each session focuses on a specific nutrition topic and builds on previous lessons to help patients develop the knowledge, skills, and confidence needed to make healthier food choices.
Whether delivered in the clinic, virtually, or during follow-up visits, these sessions provide a consistent learning experience while allowing educators to personalize discussions based on each patient’s health conditions, goals, and readiness to change.
TRAINER ACCESS NOTICE
The resources on this page are intended for educators who have successfully completed the Food as Medicine Trainer Certification process, including online training, in-person instruction, session observation, and a supervised practice session. Completion of all required training helps ensure that every participant receives accurate, consistent, and high-quality nutrition education.
INDIVIDUALIZED EDUCATION SESSIONS
Our standardized nutrition education curriculum is designed to provide staff with consistent, evidence-based tools to educate patients and support healthy behavior change. Each session focuses on a specific nutrition topic and includes everything needed to deliver the education with confidence.
Each session includes:
📖 Trainer Guide – Step-by-step instructions, talking points, activities, and teaching tips for facilitators.
👥 Patient Handout – Easy-to-read educational material patients can take home for future reference.
💻 Patient Presentation – A patient-friendly presentation with visuals to guide the education session and reinforce key learning points.
📊 Reporting :Participant documentation, goal setting, attendance tracking, and program reporting are completed
through the Kintone Food as Medicine Training and Documentation System.
These processes are covered during the required trainer certification and Kintone training.

Already completed your training?
Before teaching a session, take a few minutes to review the Trainer Orientation Manual.
It provides a quick refresher on teaching strategies, participant engagement, documentation, and program expectations to help ensure every participant receives a consistent, high-quality learning experience.
Before delivering participant sessions, ensure you are familiar with:
