This event will highlight the findings of our year-long community assessment and showcase the progress of our 4-year federal diabetes prevention grant. Designed for community leaders, nonprofit partners, and health advocates, the event will share insights, celebrate partnerships, and outline our vision for the year ahead.

Why This Matters 

Diabetes isn’t just a health issue—it’s a community challenge that touches every part of life: access to care, food security, cultural understanding, and the daily decisions people make. In South Carolina, and particularly in underserved neighborhoods across the Lowcountry, the burden of diabetes continues to grow—quietly but persistently—affecting thousands of families who lack consistent access to education, screening, and affordable treatment.

The Numbers Are Real

Over 1 in 7 adults in South Carolina have diabetes—and many more are at risk and unaware.

Our first-year community assessment found significant barriers to screening, diagnosis, and management, particularly among Hispanic, African American, and low-income populations.

It’s About Equity

Language access, transportation gaps, and food insecurity all play a role in public health

We are building solutions with the community, not for the community—and that model only works with broad support and engagement.

Collaboration Drives Change

8 partners worked together to screen residents, deliver education, train peer mentors, and provide nutritious food and preventive care.

In Year 2, we’re bringing on new partners to go even deeper—but we need your voice, your insight, and your experience at the table.
