This is a new initiative\u00a0of the Shifa Clinic\u00a0Duluth,Georgia. We received\u00a0a grant from the CVS Health\u00a0Foundation early this year\u00a0which enables us to expand our clinic\u00a0services at scope and scale. We identified\u00a0Diabetes is a major health\u00a0problem in our community and designed\u00a0special services for them, particularly\u00a0at the time of COVID-19 health\u00a0challenges. Because, Diabetic\u00a0patients\u00a0are at more risk of COVID-19 infection\u00a0and death. Our project identifies\u00a0all Diebteic patients\u00a0that are already enlisted in our\u00a0patient database and reaches out to them to check a few critical\u00a0things about their health conditions; level of diabetes control, medication compliance, and health education, among others. We support and provide them fowling services that will keep their Diabetes under control and avoid serious complications:<\/span><\/div>\n

1. Home monitoring\u00a0of blood\u00a0glucose and blood\u00a0pressure<\/span><\/div>\n
2. Regular Doctor visit and consultation about\u00a0the\u00a0condition and complication<\/span><\/div>\n
3. Assist with routine lab tests and follow up care<\/span><\/div>\n
4. Conduct educational workshops\u00a0to broaden their knowledge and understanding of the disease focusing on healthy eating and lifestyle.<\/span><\/div>\n

The project takes proactive\u00a0initiatives to reach out to Diabetic patients as opposed to\u00a0they reach out to us which often\u00a0they forget and that\u00a0leads to long periods of unmonitored\u00a0Diabetes resulting in serious complications.<\/span><\/div>\n
<\/div>\n
In our project, Emory University\u00a0School of Medicine\u00a0is also collaborating\u00a0on patient educational aspects and helping us design the educational workshop and materials.\u00a0<\/span><\/div>\n
<\/div>\n
We will\u00a0continue\u00a0to provide this special Diabetes Service to all our Diebetic patients and also to all who are at risk of Diabetes in the community. Our goal is to keep the community informed\u00a0about Diabetes and its long term health hazards and support them to adopt a healthy lifestyle to avoid the serious complications.\u00a0<\/span><\/div>\n
